1.b. Unless you're actually pulling mail down from this server, you
don't need to set the ETRN/TURN option. that has nothin to do with
outbound mail. i'd change that setting back to "Do not send ETRN/TURN"
1.c. In addition to theBasic Authentication settings, check the TLS
Encryption box. mailhop supports these secure connections, and since
you don't have to send your usernae/password in clear text, you
shouldn't.

Inside ESM, open Recipients, Recipient Policies, and open the
properties for the Default Policy. Which SMTP address is listed in
BOLD (i.e., your default address)? i'm going to guess it's the
"smith.local" address. It should be the "smith.com" address.

I think you've hit the nail on the head, here. Make the changes in the
Default Recipient Policy and that *should* get that resolved.
