SAM Error ID 12294

Can anyone please help? I'm recieving the following error on our SBS2003
server:

Event Type: Error
Event Source: SAM
Event Category: None
Event ID: 12294
Date: MM/DD/YYYY
Time: HH:MM:SS
Description: The SAM database was unable to lockout the account of ? due to
a resource error, such as a hard disk write failure (the specific error code
is in the error data). Accounts are locked after a certain number of bad
passwords are provided so please consider resetting the password of the
account mentioned above.
Data: 0000: ......

This all started occuring after we changed all the passwords on the server.
I've verified no viruses were found, and even changed the admin password back
to the original, and still no luck. I've also used the tools provided by
Microsoft, but the admin account continues to lock itself out, even though I
can still access it. Please help anyone.
