In theory, it should work from inside the network as well using that setup.
I have run across cases where pointing Entourage to the Public IP on a
server doesn't route correctly back into the internal network. What is the
internal domain name for the network? Is it different than the public domain
name?

Have your client try it the way you have it configured now. If we're lucky,
it will work without any additional modifications. If no, we can try
something different. What I have done and have done for others is the
following:
First, let's assume that your internal domain is domain.lan and your
external domain is domain.com. To access OWA from the public internet, you
use mail.domain.com.

1. Open the DNS Management console.
2. Right-click on Forward Lookup Zones and select New Zone. This brings up
the New Zone Wizard.
3. Click Next.
4. Select Primary Zone and enable the Store the zone in Active Directory
checkbox and click Next.
5. Click Next again.
6. Enter the zone name domain.com and click Next.
7. Select the Do not allow dynamic updates radio button and click Next.
8. Click Finish.
9. Click on the new forward lookup zone domain.com.
10. Right-click on domain.com and select New Alias (CNAME).
11. Enter "mail" in the Name field. The fully-qualified domain name field
should now read mail.domain.com.
12. Enter the internal name of the server in the Fully qualified domain name
field (i.e. Server.domain.local).
13. Click OK.

You should now see an entry for "mail" in the domain.com lookup zone whose
data is server.domain.local.
