No incoming mail from outside

I reinstalled ISA2000 on SBS 2003 Premium server.
The setup is:
DSL=>router192.168.12.1=>192.168.12.2Server192.168.2.2=>Switch=>192.168.2.xC
lients.

Mail works fine between machines on the local subnet. I can send mail out to
other internet sites but none comes in

The router has port 25 open from any to any and TCP port 25 is forwarded to
192.168.2.2.
I reran CEICW and ISA has rules allowing TCP port 25 in and out to/from
"default address on the enternal interface(s)" to/from any address.

Portqry and Telnet from local addresses show Exchange is listening on TCP
port 25.
Portqry and Telnet from another internet site indicate nobody is listening
on TCP port 25.

How can I find out what is blocking the incoming mail?
