Have you tried filemon, from www.sysinternals.com ?

My experience with internal 500 errors, is access denied to something, from
authentication or kerberos errors especially.
Have a look if there might be access denied to a file or dir all of a sudden
possibly. At any rate that is a great tool for troubleshooting permissions
and should be in everyone's tool kit if not already!
