According to Trend, you should install the exchange service pack 1st. I
ended up having to reinstall scanmail after exchange sp1. My situation was
perhaps unique, but it would still be better, having the option, to go ahead
and install sp1 first.

Here is a cut-and-paste from an email I got from Trend tech support...

Problem:

ScanMail issues after installing Service Pack for Exchange 2003.

Solution:

Please be informed that it is always a protocol to install ScanMail AFTER
installing the ServicePack for Exchange.

If you would install a Service Pack for Exchange, you need to re-install
ScanMail. This is because ScanMail requires a ServicePack installation
before ScanMail is installed.

When you have applied the ServicePack, ScanMail was not updated of the
ServicePack 1 update.

To solve this, un-install ScanMail and re-install ScanMail.

This is a known issue.
