yeah I had this problem, SBS std, Trend SMB and Exchange using POP
connector.
It started (I think) when one of the Servers RAM Dimms died. Mails then
were getting stuck...I troublehooted it down the the Scanmail component
of Trend and turned it off (I guessed it was due to low RAM - 256MB)
Otherwise, the only thing to clear the queue was to reboot the server
Anyway, left trend off for a week and everything was sweet. When the
Dell man came to replace the RAM (turned the server off by holding the
power button in by the way) I turned scanmail back on and everything was
fine.
So in my case I felt it was due to low system resources... maybe check
your RAM (memtest86.com) swap file, that sorta thing. Do an update of
all the trend engines and definitions too - cant hurt
