A couple of questions:

1) Can you telnet to port 25 on the external IP address of your server
from a computer outside of your SBS network? This will tell you if your
router/firewall/ISP is blocking inbound port communications. For complete
instructions, see:
153119 XFOR: Telnet to Port 25 to Test SMTP Communication
http://support.microsoft.com/?id=153119

2) Are you hosting your own DNS or does a 3rd party host your DNS? It can
take some time for new records to propogate. If you are hosting your own
DNS, can an external user perform an NSlookup against your server? The
steps to do this are outlined in:

203204 XFOR: How to Obtain Internet Mail Exchanger Records with the
http://support.microsoft.com/?id=203204

For both tests, it is critical that you test with a machine outside of your
network.
