I am running Windows 2003 X64 R2 with XP Pro SP2 clients in a domain
model.

We have user folders with access specifically controlled using share
and NTFS permissions so each user can only access their own folder.

Folder permissions are as follows:

Folder = User1
Share Perms: User1 Full Control
NTFS Perms: User1 Full Control

All others have been removed.

Everyone initially could access their folders. All of a sudden all
but two users could successfully access their user folders with the
above permissions. The two users received the following error when
attempting to access their user folder: H:\ is not accessible.
Access is Denied.

Items of note:

1. If I blow out the XP profile on each user's PC that is
experiencing this issue and create a new profile the user CAN access
the folder as expected. Sometime after the profile is created the
error appears.

2. If I add the everyone group to both the Share and NTFS
permissions, the user can access the folder.

3. If I only add the everyone group to either the Share OR NTFS
permissions, the user CANNOT access the folder.

4. Stopping sharing and re-sharing the folder has not effect.

5. We have OSX machines on the network so SMB digital signing is
disabled for both the client and server.

Did you add System to NTFS perms? That should be in there with FC. Try that.
