They can also come from DHCP or DNS. It don't know the steps to reproduce it
myself,...at least not by memory. But those two are actually the most
popular way to do it since the GP way has only been around sicne 2000 Server
came out, while DHCP and DNS were used even back in the NT era.

Another way is for the proxy settings to be designed into a customized
installation of IE which can be done with IEAK.
