How to customize error 403.htm?

We have set up IIS with a number of IP restrictions. We do not know all the
IPs of our group, and so we want to send IPs not in the group to a customized
page where they can apply for inclusion.
Right now, the only thing a non-admitted IP will see is the local browser's
403.htm error message, which resides here:
://C:\WINDOWS\system32\shdoclc.dll/http_403.htm
The Custom Errors tab in the IIS admin interface does NOT list error 403.htm
- although that page is in the directory, and it appears impossible to
customize using the IIS interface.

Note that this question is specifically about error 403.htm - not 403-1.htm,
403-2.htm etc. which are easy to customize.
Does anyone know of a workaround to stop users being given the file from
their local machine and given something more helpful?
