Indeed. He didn't say which proxy HTTP server he is using. In squid,
for example, one of the better tricks is to employ a URL redirector such
as "adzapper" from <URL:http://adzapper.sourceforge.net./>. This
rewrites undesirable URLs in incoming requests so that they become one
of the objects from <URL:http://adzapper.sourceforge.net./zaps/> (or any
other set of objects of one's choice).

... and affects all softwares running on the machine, including those
that are _not_ web browsers and web browser plug-ins, thus tackling
Microsoft Worms and the like too. The logical extension of the scheme,
once the number of workstations becomes too great, is to publish the
same data on a local content DNS server instead, and to configure one's
proxy DNS server to query it using the normal "split horizon" mechanism.
The DNS was, after all, created to solve this very scalability problem
with HOSTS files.
Mark appears to be concentrating upon web browsers and web browser
plug-ins in his second message. One of the approaches to that is to use
a PAC script to change from whence the browser fetches particular
objects. There are several PAC scripts available that will redirect
requests for advertising banner image objects and the like.
