It could be all kinds of things, from somebody else keying in the wrong
credentials, to the user being logged into another machine in the
environment with a mapped drive (or some other open resource) and changing
the password elsewhere.
Check out the ALTOOLS on microsoft's website for a way to troubleshoot this.
