It's possible that there's some stale connectivity data on your Xbox 360.
Please follow the below steps to clean everything out and start fresh.
First, do the following on your Media Center PC:
1) Uninstall the Media Center Extender software by going to Add/Remove
Programs under the Windows Control Panel
2) Reboot your PC
Next, do the following on your Xbox 360:
1) Go to the Xbox 360 dashboard-->System blade (purple
area)-->Computers-->Windows Media Center and choose "Disconnect"
2) Back out to the top-level of the dashboard, go to the Media blade (blue
area), select Windows Media Center and continue forward to get a new 8-digit
code...write it down
Finally, return to your PC and do the following:
1) Go to www.xbox.com/pcsetup and re-download the Media Center Extender
software
2) When prompted, type in the 8-digit setup key you wrote down above
