Bearing in mind this is from my perspective (as a DVB-T user). Here's
all I think it needs:

1. Type of signal (FreeView, Cable, Sky, Analogue). BTW, don't use
"DVB-T" in the UK since most people don't call it that and would be
confused if you did.
2. Post code - so you can usually work out which transmitter is
involved.
3. Transmitter (if known) - so you can be sure, since many people can
receive signals from more than 1 transmitter.
4. A reasonable text field where someone can type a description of the
problem.
5. A voluntary email address field so that you can get back to people
to request any further information if necessary.

To report a problem with the guide from within MCE, I should also like
to have the reporting integrated such that you can right click on a
guide item (this is the one at fault), select a context menu to report
the error, fill in a text field and press Send.
