Random Mute and Volume Display

After 4 months my MCE has decided to display the volume level every few
minutes and to switch to mute on its own. It has also started to display the
volume flag and show mute whenever I am pressing a button on the remote.

Anyone have any idea what might have changed to cause this and how to change
it back?
