You can either use the IP address, or you can use a service such as DNS2Go, www.dns2go.com. DNS2Go involves running a client on the computer you wish to connect to. The client sends your IP address to DNS2Go's servers, and its translated to a host name, http://yourhost.dns2go.com for example.
Your router needs to be configured to forward port 3389 to the machine you wish to connect to. Forward port 3389 to LAN IP 192.168.0.xxx. You'll have to consult your router's manual for information on how to open/forward ports.
