Your issue is with the volume. It is /not/ set as full volume.

I got your message, and I found the problem. The volume for the background sound was set to -9640 which is essentially off. The volume setting ranges from -10000 to 0 with -10000 being off and 0 being full volume.
OE doesn't let you explicitly set the volume for the sound. Instead it picks up the current volume setting. So before you insert the background sound you need to make sure your volume setting is correct. Double click the speaker icon on your task bar. For a MIDI file it will use the volume control labeled "SW Synth" or something similar. It may vary depending on your sound card.
One additional thing, the MIDI file you used has about 5 seconds of silence at the beginning, so there is a pause before it starts playing. I don't know if that was intentional on your part.

I uninstalled AVG and decided to experiment before reinstalling it.
I sent one message with a wav file and another with a midi. In both cases
Outlook Express saw only the text, no sound, when I downloaded a message. In
both cases, when I downloaded a message in Outlook, the music did not play
(not embedded?) but the file was attached to the message.

AVG is one of the AV programs where simply unchecking e-mail scanning may
not be enough.
Reinstall AVG and choose Custom Mode. Uncheck E-mail Scanning when you see
that option. Just unchecking it in the AVG Security Center usually causes a
conflict with the Windows Security Center.
