The CSV file should be located in the same folder where you installed OETool. Another option you can use, which may be easier while viewing the Header list, is just highlight the page (Ctrl+A) then Ctrl+C to copy it to the clipboard and from there to an open example of Notepad and save it in a convenient location as a CSV file.
