look at HKLM\Software\Microsoft\Outlook Express
See what value BlockExeAttachments has

0 should be off
1 should be on.

Makes me wonder if something is setting that value back such as a tweak
Windows type program.
WinXPManager comes to mind. A client was using it and would change the
setting and it would change back, turns out thats what was setting it back.
