Yes, "Smooth-scroll list boxes" is checked (as are all the other options).
But the scrolling that I do get is not smooth but jittery, like it scrolls 6
pixels every 6 vertical retraces instead of one pixel each retrace (this is
not meant as an analysis of how the feature is implemented but only as a
description of what it appears like).

Hit your "Windows 'key' + "Pause Break" simultaneously.
Click on the "Advanced Tab" > "Performance" - 'settings' - button.
Under 'Visual Effects' - what settings are applied?
"Smooth Scroll for List Boxes" should be checked.
