File download not working for some

Ive created an icon to be placed on a webpage for download. The weird
thing is that when some people click on it it prompts them with "File
Download" dialog box which is what it should do. However, some people
are not getting prompted with dialog box and some are even getting
permission errors. I have checked the permissions on the folder where
this file sits and they are set correctly. Any ideas what could be
causing this?
