Mysterious Entries in Address bar

I have started to accumulate URL's in my Address bar that are
unfamiliar and in many cases not even valid links even though the
addresses seem property formed.

Is there any way an address can be added to the bar without a user
typing it in?

I run Spybot regularly but it isn't finding anything.

Many of the entries are adult in nature even though I'm running
ContentProtect nanny software, and I'm pretty sure my 2 year old isn't
hacking the password.

Any ideas???
