For the tools menu:

Use Regedit.exe and remove the item. It should be under here:

HKEY_LOCAL_MACHINE\Software\Microsoft\Internet Explorer\Extensions

For the right-click menu:

Navigate to this key and see if your offending item is there in the
list of sub-keys. If so, then remove it.

HKEY_CURRENT_USER\Software\Microsoft\Internet Explorer\MenuExt
