Drive disappears after CHKDSK

I have run CHKDSK on an NTFS volume. The report below is what was generated
in the Event Viewer.
The problem is that after Windows Server 2003 reboots the drive is not
available.
Under the Disk Management tools the drive appears as unallocated.
What's the deal?
How can I recover the data and volume structure of this drive? Is it lost?
