Ok, I'll ask a dumb question, but are you certain that you've got the
speakers plugged into the correct jack? Check the manual to be certain. If
you can't find the manual, you can download it from Creative's download
library.
Does this card show up in Device Manager? Is there a yellow "?" mark next
to it?
Also, you mentioned that this is a Creative card, but not which model. If
you go to Start/All Programs/Creative do you have an icon anywhere within
this folder (might be a in a subfolder) for Creative Restore Defaults? If
you've got an Audigy 2 card it would be under Sound Blaster Audigy 2.
You could also try reinstalling the drivers for you sound card off the CD
that came with it.
