Well, it's only a guess, but there is a file called comdlg32.dll that has all
the 'common controls' like that in it. I suppose that your app may have put
another version in the path and that one is fighting with windows. I would
try searching for hidden files and folders and comctl32.dll. If you hit one
outside of the system ones, like service pack, uninstall, system32, WinSxS
and dllcache, it may be a bad version.
I don't think reinstalling the system versions is going to do anything. If
nothing else shows up, I would still suspect some file or icon problem.
Another likely candidate is a hidden file named 'Thumbs.DB' in each folder.
No harm in deleting that to test.

appears to be the same dialog box (size, location, default content) that is

To" commands (with different title frames.)

Additional info... while waiting for the dialog to populate, there are no
indications that the drive is being hit (lights not on) and the processr
continues to operate betwen 2 and 3% according to the performance monitor.

I have no problem running MSOffice apps, opening files within them, or
opening files using the file manager and "Open" or "Open With" commands.

The same six-minute lag occurs when I try to "Save" or "Save As" inside the
Digital Imaging application, but not inside the Office applications. Can
anyone give me an idea where to look to start to resolve this issue??
