Okay. That means that your hard drive has been divided into 2
partitions at some point.

You should be able to do a clean install onto your drive C: by
following the instructions at
http://michaelstevenstech.com/cleanxpinstall.html

Then when that is finished and up and running you can format the D:
drive to get rid of the extraneous XP Home install.
