I spent a week tracking down a Code 12 problem on a Nvidia Video Card.
It turned out that the CODE 12 was a False Positive.
The root of my problem was Power and / or Bad Capacitors on
the Motherboard.
You know it's one these problems when every once in a while you try
to start your Computer (cold) and the Power Supply automatically
shuts off. Then, you can't start it again until you physically
disconnect the Power Supply, plug it back in and try it again.
Sometimes it took 3 or 4 trys to get started. However, once the
computer is warm, it boots / reboots just fine.
Once I got the computer started, I let it run overnight, then I
rebooted and the Code 12 was gone and everything worked just fine
unless I turned the PC off and let it cool down. Then you have
to let run for a long while again, reboot and you are fine.
Possible Solution 1 - Try a Higher Wattage Power Supply.
Possible Solution 2 - You have Bad Capacitors on your Motherboard.

My Solution 3 - Live with it for now by letting the PC run
overnight and then reboot. The Device is automatically recognized
and the code 12 is gone. I did absolutely nothing other than let
it run a long time and then reboot.
However, keep this in mind. This PC had not run in more than
1 year and it would not boot. It had a 250 Watt Power Supply
in it. I replaced it with a 350 Watt. PC then booted and ran
fine, however, I got the shutoff problem after leaving it set
overnight between boots. I then changed to a 450 Power Supply.
Solved the shutoff problem, but still had the code 12 problem.
Let it run 12 hours, then reboot, works fine. Get it working
then do not shut it off! You are OK with reboots as long as
you do not let it cool down. My permanent solution will be to
get the MotherBoard Caps replaced. This PC was a Medion 1453.
This is known as a high end eMachines Computer.
If the Capacitors are bad, you will start going through Power
Supplys every 4 - 6 months. The Bad Capacitors are Power
Supply killers.
