My screen suddenly turns blue, counts up, then comp restarts...

hi everyone...
i've had this problem for a VERY long time and i don't know how to fix it.
everytime i'm on my comp, it will eventually turn blue saying a bunch of stuff...and at the bottom, it says DUMPING PHYSICAL MEMORY TO DISK and numbers will start counting up. i took some pictures of my screen so people can see what i'm talking about:

if someone could PLEASE help me...that would be great.
if it's a hardware problem, also please let me know.
