disk is not initializing

i had a dynamic disk failure, which i described in another post
here[1]. i believe it's just a software problem, because R-Studio (a
data recovery software) was able to access the disk and read some of
the files off it. now i want to reformat the disk as new. so following
the advice i was given on the other thread, i did

diskpart
select disk 1
clean

then in Disk Management, i right-clicked the disk and selected
Initialize Disk, but the disk is still in Unknown/Not Initialized
state. is there anything i need to do to create a volume on the disk
that i can partition?
