There is a procedure here. This procedure causes the USB ports to be
re-enumerated, and presumably any devices that were previously
screwed up, would be forgotten.

includes an Intel chipset, plus a NEC add-in USB2 capable PCI

The top link is not so much a "fix" as such, as a generic solution
