A good way to test your hardware is by booting into a rescue system like
Knoppix or a Bart's PE. If the usb ports don't work there, then you know
they have failed. If the ports are on the motherboard and they have failed,
the only way to fix this is by replacing the motherboard. If everything
else is OK, a decent workaround would be to install a PCI usb controller
card (very inexpensive).
If the ports work OK in the rescue system, then go back into Windows and
delete them from Device Manager. Reboot and Windows should find and install
them again.
