........First a transient establishes a complete electrical path to earth ground. Then something fails in that path.

Interesting, hadn't thought about it that way before.

It's easy to see how the cases of equipment is grounded by the recepticals, but how can the hot legs be grounded too? My neighbor has one of the Pepsi sized cans installed in his breaker box that's supposed to fail if struck, is that the answer? I've noticed the power company runs ground wires down their poles around here just about every third pole. They take grounding serious.
