Two possibilities that I can think of as well:
1) Make sure that they are all on HiSpeed in the BIOS. It is possible
to have half USB 1.1 and half USB 2.0 (but that would be an odd option
in the BIOS and definetly not a default).
2) The pins are set up wrong on the motherboard for the USB connector.
This would only pertain to the front panel USB connectors. If you have 2
slots on the front and one works, but the other doesn't, check and make
sure both are connected the same.
