It has nothing to do with how many times you've installed an OS on that machine. Failure to install an operating system usually is because of faulty hardware:
1. Bad OS install disk
2. Bad CD/DVD drive
3. Bad RAM
4. Bad hard drive

Whether hardware is new is irrelevant; in fact if hardware is going to fail it will usually do so quite quickly (sometimes even right out of the box) or go for years.
