Usually hardware failure or possibly file system corruption. Use the manufacturer's diagnostic tools to test. I'd start with the disk drive, drive controller and memory.

Last time i scanned my HD (NTFS), there was a big line of index
entries deleted from file $9, saying something about repeated entries.
I switched it off at once and restarted it.
When I logged into windows, it popped up that my recycle bin was
corrupted. A few files was lost.
The next day, I let it go on. The numbers went up, and the screen
flashed. I waited for a ten minutes, and still flashing. I switched
it off. After restarting, my boot sector was empty. HD half empty.
