XP has a built in account called Administrator, that's why you can't create one with that name. Once another account with admin privileges is created the built in Administrator account is no longer visible. To access it for XP Pro at the Welcome Screen do ctrl-alt-del twice, enter Administrator as the user name and the password assigned to that account when XP was installed. For XP Home it is only accessible through safe mode. Boot into safe mode and Administrator will appear on the Welcome Screen. Normally the password is unassigned (blank) so just click on it.
You should not use the built in Administrator account for day to day activities in XP. That account should be set aside with a strong password. For XP Home in safe mode assign a password to the account. Keep that account aside to be used if something damages the normal working account. Have at least one other account with admin privileges and any number of limited user accounts for normal activities.
