Explorer folder list bug

Win XP Pro, SP2, all latest automatic updates.
Additional updates: KB894391 (fix for svchost.exe crash).
Also using Tweak UI.
This is a bug a experience regularly but with no consistency in time,
directories, or other running programs. The folder list just spontaneously
generates extra items, and sometimes duplicates the entire directory tree as
it is currently displayed.
I took a couple of screenshots:

A search of MS support didn't turn up anything definitive. Can anyone help?
