AFAIK the registry does not inherently maintain itself.
If anything is maintained it's the result of the de-install routine of any
given application upon deinstallation.
Should it be cleanr?
Maybe.
Should you use a registry cleaner?
If you don't know yourself how to, don't rely on the 'wisdom' of someone
having put together a registry cleaner program.
