I have seen this happen once when the analog audio cable from the CDROM
drive to the motherboard was pinched between the motherboard and the video
card. In a quest for neatness someone had run the cable underneath the video
card between the back of the computer and the AGP slot. Removing the cable
solved the problem as the cable is not really needed in most systems. By
default Windows XP uses digital sound.
