Startup folder items run twice

Can anybody assist ?

I have a weird problem. I have an XP sp2 w/s on which the
startup folder from the user account had been removed.
I created a new Startup folder in C:\Documents and
Settings\UserName\Start Menu\Programs and placed the
startup app in it. It doesnt run. I tried with standard
apps (notepad) just to make sure it wasn't related to my
application (msaccess) - it didn't run either

I thought I would get around it by placing the startup
app in the 'all users' startup folder instead. (removed
folder altogether from user). It now starts twice ! I
have tried with several apps and they too start twice.

Any ideas ?
