1. Lost from where? Desktop, Start Menu, Quick Launch, etc?
2. Use accordingly:

If you are using TweakUI go to Explorer/Uncheck Allow Logoff on the Start
Menu

If you're running Pro, click Start, Run and enter GPEDIT.MSC Then navigate
to User Configuration, Administrative Templates, Start Menu and Taskbar.
You'll see the entry to Remove Log off.

If you have XP Home:

If you prefer to do it manually, you can see the following KB Article for
the instructions and settings.
