I believe you may be confused about CPU Idle Process running at or near
100% - this is CPU Idle time = doing nothing.

As far as the dusty fan - yes clean it up. That won't affect CPU usage
numbers but it will affect CPU life and system stability. By the way,
most modern CPU running temperatures are rather hot to the touch anyway
but clean out the dust and make sure all fans are running.
