Thanks for your attention here, Kelly. I got the video clip from Sam and noted that the dynamic open-with menu does not expand though the Openwithlist entries are present (and the sub-menu present, but not popping up). Sam says that the problem is seen with many (or all) filetypes except the Unknown filetypes. I suspected the entries under Allfilesystemobjects, but not very sure about that nor I could reproduce the problem. Interestingly, he says the problem not seen while logged on as Administrator. Sounded a complex problem, to me :o(

Am just skimming through all of this now...
Examples for .jpg
1.  If you are clicking on Picture and Fax Viewer, nothing will happen 
unless:
regsvr32 shimgvw.dll is registered under Start/Run.  And also if Open has 
been removed.
2.  In your case, not sure, it seems you don't have Open or Preview, which 
would seem you deleted the association that prevents Picture and Fax from 
stealing focus, which should be located here:
HKEY_CLASSES_ROOT\SystemFileAssociations\image\ShellEx\ContextMenuHandlers\ShellImagePreview
@="{e84fda7c-1d6a-45f6-b725-cb260c236066}"
Or to remove Image Preview completely:
HKEY_CLASSES_ROOT\SystemFileAssociations\image\ShellEx\ContextMenuHandlers\ShellImagePreview
3.  If this is the case, under .jpg create a new action named Preview and 
set Open as default.
4.  Other thoughts:  Have you have tried to change the Edit improperly, 
perhaps?
Go to Windows Explorer/Tools/Folder Options/File Types.  Select an image 
type (jpg).  Click on Advanced then New.  Name for Action type in: &Edit. 
Under Application used to perform action: Browse to the 
application/software.  Set it as default. Ok your way out.
5.  What happens when you right click a .jpg then go to Properties/Change?
6.  Also some third party apps, cause this:  PSP (for one) when File Options 
do not contain a default action.

This setting allows you to control whether the checkbox to save the "Open 
With" program is available when an unknown file type is opened.
HKEY_CLASSES_ROOT\Unknown\shell\openas\command
and
HKEY_CLASSES_ROOT\*\shell\openas\command
To enable the checkbox set the (Default) value to:
rundll32.exe shell32.dll,OpenAs_RunDLL %1
Or:
%SystemRoot%\system32\rundll32.exe 
%SystemRoot%\system32\shell32.dll,OpenAs_RunDLL %1
Return to the desktop and press F5 for the change to take effect.

Option:  Go to Start/Run/CMD and type in:  assoc
Copy the line for the association you wish to restore.  A Command Prompt 
window will open. Right click in this Window and select Paste. Then press 
Enter.
