OK, sorry :-)

It's XP Home.

And I do know a fair bit about Windows (I'm a software developer), I
just don't always express myself very clearly (I'm a software
developer) ;-)

It's the former. Normally, when I click on an application (ie it gets
focus), it goes to the top. When I click on another application, that
goes to the top. When the problem shows itself, clicking on the
second application sets focus to it, but leaves the first application
on top of it, hiding the other one.

The applications that were running when it occurred just now were
Explorer (2 instances), Internet Explorer 6 SP1 (4 instances), Agent
1.93, and MailWasher 2.0.

I'm monitoring the situation to see if I can narrow it down at all.
But I've got exactly the same open at the moment with no problem. And
I never had any difficulties of this sort under W98 or W2K which is
what led me to suspect XP.
