sounds like a problem writing user accounts to the registry properly.
when you reinstalled windows, did you format the hdd?
if not try a defrag and a chkdsk /r
if you did then im thinking on shutdown your registry is not saved properly
try restarting in safemode a few times and see if it happens there
lastly it could be corrupt RAM modules causing this.
