Hi Terry,
I have not a clue why this happens.
Try two things.
1. Use new device if you have one available.
2. If you are using some third party drivers remove them.
Try to find some info from following search:
http://groups.google.com/groups?hl=en&lr=&ie=UTF-8&c2coff=1&q=0xc0000221&btnG=Search&meta=
Although I don't think that they are related.
