Brad,

The "Everyone" and "SYSTEM" groups are missing in the list. Right-click the
"User Account pictures" folder and choose Properties. In the Security tab,
add SYSTEM to the list, and assign "Full Control" to it.

Using AccessEnum is a nice way to view file / folder / registry permissions.
