I imagine the full-up Adobe Acrobat (not just the Reader) has those
capabilities. The Arizona state income tax forms online come in the
form of a PDF file that not only has fillable fields, but performs
calculations based on the entries in those fields.
Spend a little time on the Adobe website, you'll probably find what
you're looking for.

If you had it in a JPG format, you can load it into Word and set the image properties to allow text to write over the image, not around it. Now you simply have to use tabs, spaces, returns etc to place text in the appropriate boxes. Also make sure the image is in the background not foreground. Lame, but it works.
